Post acute services include long-term acute care hospitals (LTACH), skilled nursing facilities (SNF), inpatient rehabilitation facilities (IRF), home health agencies, assisted-living facilities, and some outpatient services such as physical therapy, occupational therapy, and speech therapy.
Long Term Acute Care Hospitals (LTACH)
An essential entity within the comprehensive healthcare continuum. Long term acute care hospitals provide medically complex patients with the necessary level of care for an extensive length of stay (generally 20 – 30 days).
Skilled Nursing Facilities (SNFs)
Skilled nursing facilities, or nursing homes, are senior continuing care facilities where medical coverage is available 24 hours a day to manage, observe and evaluate acuity progression by skilled nursing and/or rehabilitation staff.
Inpatient Rehabilitation Facilities (IRFs)
Inpatient rehabilitation facilities deliver specialized and comprehensive levels of patient care service programs (physical, occupational and speech) in an effort to maximize patients’ functional abilities and quality of life.
Home Health Care
Home Health Care provides the elderly with medical limitations an independent living environment with diverse services in effort to delay the need for long-term nursing care.
